Many companies have a
return-to-work program (RTW) or transitional employment that
accommodates an injured employee who has work restrictions. Please
share your RTW program with the case coordinators and the
occupational medicine physicians. Medical professionals will work
with your company to assist in providing information to help you
design an individual RTW plan for your employee.
The medical
professional will identify on the Illness/Injury Report form the
safe parameters in which the employee may return to work. The most
important part of a successful early RTW program is planned and well
communicated gradual work transitions. In some cases, it may be
helpful to provide the physician
a copy of the developed plan on the Transitional Work Program

It is also important to provide the physician with necessary
information on the physical demands of the job. This information
helps make better treatment and return-to-work decisions.
If you have a specific modified job for your injured employee,
complete the Transitional Work Abilities
form
(PDF) to help identify the physical
requirements of the job. The physician will review the form and
provide a medical recommendation.
A Transitional Employment Progress Report
(PDF) helps keep the
physician informed on the status and progress of the injured
employee in the return-to-work process. The information also assists
the provider in monitoring the treatment progress and helps to make
important decisions or adjustments to the treatment plan.
It is important to notify the provider if your
company has limited or no ability to provide temporary modified work
options. In some instances work conditioning may be considered for a
safe, timely return to work.
